Permits can only be issued to residents within the town limits. Please bring in the completed form for application from the Physician or a prescription from the physician and the License or Non-Driver ID for the person it is being issued to showing residency. If the person receiving the permit does not have a NYS License or Non-Driver ID you must bring in a bill dated within the last 30 days showing their name and address to provide proof of residency. We cannot accept PO Boxes as valid residency.
